Birthday Party Host Payment

Jason LeMaire is taking the lead on paying our Birthday Party Hosts, if this is the first time you are submitting this form you will need to provide Jason a W-9 and Bank Account Number and Routing Number to be reimbursed through ACH payment. We will no longer be providing checks. A Blank Copy of a W-9 to be downloaded is at the bottom of this page. You can email it or give it to Jason. Jason’s Email is below.
